Plum Creek Medical participates in most insurance and managed care plans. We will need a copy of your insurance card at each visit in order to submit your insurance claim.
Services not covered by insurance, including co-pay, deductible and co-insurance are due at the time of service. We accept Visa, MasterCard, American Express and Discover for your convenience.

Referrals
Most insurance companies prior to any procedure or specialist visit require preauthorization. The typical referral will be directed by the primary care physician (PCP) to the correct network provider. The patient must call the designated provider for an appointment and then coordinate with our referral specialist to initiate the appropriate referral. Retroactive referrals are no longer possible unless an emergency is involved.
